• S007 - Drug Regimen Review
• S013 - Falls with Major Injury
• S024 - Discharge Self-Care Score
• S025 - Discharge Mobility Score
• S038 - Changes in Skin Integrity Post-Acute Care: Pressure Ulcer/Injury

Like other QMs, we have historical performance and assessment detail. QRP QMs are defined by the Medicare Part A Stay. For this reason you will see an entry and a discharge assessment for each stay. You are also likely to see a resident appear more than once in a quarter which is unlike Short or Long stay QMs.

Where to locate the new QMs

These new QMs are available on the Quality Measures page under the Quality tab in SimpleAnalyzer™. We've also made these QMs easily accessible with a new "QRP-based" QM filter.

Breakdown of new QRP quality measures

S001 - Functional Assessment/Care Plan - This measure reports the percentage of PPS residents with an admission and discharge functional assessment and a care plan that addresses function.

S002 - Percent of Residents or Patients with Pressure Ulcers That Are New/Worsened - This measure reports the percentage of Medicare Part A SNF Stays (Type 1 SNF Stays only) for residents with Stage 2-4 pressure ulcers that are new or worsened since admission.

S007 - Drug Regimen Review -  This measure reports the percentage of PPS residents with a drug regimen review conducted with follow-up for identified issues.

S013 - Falls with Major Injury - This measure reports the percentage of PPS residents experiencing one or more falls with major injury.

S024 - Discharge Self-Care Score - This measure estimates the percentage of Medicare Part A SNF Stays that meet or exceed an expected discharge self-care score.

S025 - Discharge Mobility Score -  This measure estimates the percentage of Medicare Part A SNF Stays that meet or exceed an expected discharge mobility score.

S038 - Changes in Skin Integrity Post-Acute Care: Pressure Ulcer/Injury -  This measure reports the percentage of Medicare Part A SNF Stays (Type 1 SNF Stays only) with Stage 2-4 pressure ulcers, or unstageable pressure ulcers due to slough/eschar non-removable dressing/device, or deep tissue injury, that are new or worsened since admission.
